IRS 1095-C FORM INFORMATION FOR INDIVIDUALS WHO HAD MEDICAL COVERAGE WITH D51 DURING THE 2024 CALENDAR YEAR.
The IRS requires District 51 to report whether an employee and dependents (if applicable) had health coverage in 2024. Therefore, employees receive Form 1095-C, which details whether they were offered health coverage by District 51 and during which months.
The Paperwork Burden Reduction Act, signed into law Dec. 23, 2024, has amended the requirement for employers to mail a paper copy of the form to every employee. With this new legislation, paper forms will not be mailed to employee homes unless requested.
The IRS does not require a copy of your 1095-C form to complete a tax return.Forms are now available online through the Workterra portal (instructions linked here) or requested by:
- emailing benefits@d51schools.org
- mailing Human Resources at 2115 Grand Ave, Grand Junction, CO, 81501
