To better support you, your school and teachers would like to ask a few questions about how you're feeling and how we can help.

Your responses will only be visible to your teachers, school leaders, or approved District 51 personnel. They will not affect your grades and will not appear on your report card.

Please answer honestly—there are no right or wrong answers. You can skip any question you do not feel comfortable answering.
