To better understand your school experience and how we can improve, your teachers and school leaders would like you to answer a few questions about how you feel at school, your connection with others, and your thoughts about your teachers and classes.

Your responses will only be visible to your teachers, school leaders, or approved District 51 personnel. They will not affect your grades and will not appear on your report card.

Please answer honestly—there are no right or wrong answers. You can skip any question you do not feel comfortable answering.
